Power of Attorney Forms

A power of attorney form is a document that authorizes another person to act on behalf of the individual granting the power of attorney. The person granting the power is called the Grantor and the person receiving the power is called the attorney-in-fact.

Almost anyone of legal age and mentally competent can be the attorney-in-fact.

Our contracts and agreements are drafted by licensed attorneys and updated regularly.

Order 24 hours/day, 365 days/year on our secure servers and have immediate access to your forms or software.

Our power of attorney forms are not generated by software programs and you do not have to fill them out online, nor give out your personal information online. These forms are drafted by a licensed attorney and you may download them to fill out in the privacy of your home or office.

These four types of power of attorney forms will cover many situations that people will encounter in their personal or business lives.

1) Specific or Limited Power of Attorney authorizes the attorney-in-fact to act on specific matters. For example: The selling or buying of personal property, handling of special business transactions, taking care of certain banking transactions, or any other tasks or transactions you would like done.

2) Durable Power of Attorney for Health Care is a form used exclusively for authorizing one person to make medical and health care decisions for you when you are no longer capable of making them for yourself as determined by a licensed physician.

If designated, your agent will be able to make all health-care decisions including the type of treatment, location of treatment, and in addition, the right to refuse or decline life prolonging treatment to keep you alive. Any special instructions can be included in this form so your wishes are followed exactly as you would like them to be.

3) Durable Financial Power Of Attorney is a legal document that gives another person partial, or in some cases complete control of another persons finances. You can choose to have the durable power of attorney for finances become effective immediately, or if you ever become incapacitated and cannot make decisions for yourself.

The financial power of attorney will keep you in charge of your own destiny and will let you to determine who will have the legal authority to act for you should the time ever come when you cannot make decisions for yourself. Unless such incapacity occurs, you continue to retain full control over your affairs.

If you become incapacitated, the person you have appointed will take over your affairs and act within the guidelines and restrictions you have already established. The attorney-in-fact can pay bills, deposit checks, handle taxes, sell stocks, sell real estate, invest in securities - in short, everything that you could otherwise do for yourself.

Different states have different requirements for power of attorney forms. Our power of attorney forms have been drafted by a licensed attorney to meet or exceed requirements for all 50 states except Pennsylvania.

4) A Revocation Form is the form used to revoke or discontinue any of the power of attorney forms listed above. We include this form free of charge with the purchase of any of our power of attorney forms.

These attorney-drafted power of attorney forms come in an easy-to-use rich text format and can be used with most any computer. Power of attorney forms are simple to prepare. Fill out the form, sign in front of witnesses and notary, and it's finished.

100% money-back guarantee if these forms are not accepted by any financial, business, or health care facility in any state.
